Call 281-491-4545Complete new patient formsChoose a 30-minute consultation request on Monday, Wednesday, Thursday, or Friday. Staff will contact you to confirm and collect the $25 reservation fee. If you attend your appointment at the scheduled time, the fee is refundable or can be applied toward your procedure deposit. The request is not confirmed until staff collects the fee.

Prepare for your visit
Bring a photo ID, insurance card if applicable, a current medication list, and any referral notes or recent imaging you have. If you need help with parking, the entrance, or accessibility, call our office.
For severe facial swelling, trouble breathing or swallowing, uncontrolled bleeding, or major trauma, seek urgent or emergency medical care.
